Fraudulent claims

We will not accept deliberate manipulation and fraud in order to obtain these payments, including failure to self-isolate and put others at risk. Anyone caught falsifying their application to gain payment will face prosecution and any funding issued will be recovered.

You can use our online form to apply for a Test and Trace support payment if:

  • you've been told to self-isolate because of coronavirus
    or
  • you're the parent or guardian of a child who has been told to self-isolate from 8 March 2021, and you need to take time off work to look after them

You need to claim within 42 days of the first day of the self-isolation period.

Before you start

If you're filling this form out for yourself or someone else

You'll need:

  • to provide information about your employer or details about your business if you're self-employed
  • to provide the start and end date of your self-isolation period
  • your National Insurance Number
  • proof of your NHS Account ID for coronavirus or proof of the Account ID you've received from NHS Test and Trace to self-isolate
  • a copy of your most recent bank statement that shows all transactions for a month including wages, benefits and allowances being paid in. The statement should be no more than 2 months old
  • a copy of your qualifying benefit award letter if you receive benefits
  • a payslip or evidence to show that you're self-employed

If you're applying because your child is self-isolating

You'll need to provide information about yourself, including:

  • information about your employer or details about your business if you're self-employed
  • your National Insurance Number
  • proof of your NHS Account ID for coronavirus or proof of the Account ID you've received from NHS Test and Trace to self-isolate
  • a copy of your most recent bank statement that shows all transactions for a month including wages, benefits and allowances being paid in. The statement should be no more than 2 months old
  • a copy of your qualifying benefit award letter if you receive benefits
  • a payslip or evidence to show that you're self-employed

You'll also need to provide information about your child, including:

  • contact details for the education or childcare setting
  • a copy of your child's EHC Plan if they have one

The information you provide will be checked with your child's education or childcare setting.

Note: if you do not submit the evidence, your self-isolation support payment may be delayed.

The council primarily use your data for administration of the Test and Trace support payment. This may be shared with other Local Authorities and government departments in accordance with our organisational privacy notice. We never sell your data to third parties or use it to market to you without your consent.
