You can use this online form to request a school place for a looked after child.
You must be a social worker to fill out this form.
You must also read the guidance notes.
If the child has an EHCP
If the child has an education, health and care plan (EHCP) then you cannot use this form. Instead, you’ll need to get in touch with the EHCP team by emailing specialeducation@stockport.gov.uk.
Before you start
Make sure you have the following information, as you’ll need to tell us:
- the child’s current year group
- if the child is on the SEN register
- which local authority is responsible for the child, and when they came into the care of the local authority
- about the child’s care placement
- the child’s legal status
- contact details for the child’s social worker
- about the child’s current school
- the reason for the transfer request
- information about any exclusions or suspensions that the child has had
- the options that the child has chosen to study at GCSE
- about any agencies that are involved
You can also attach any of the following supporting evidence if available:
- CAF
- PEP
- information to show levels of attainment
- attendance record (past 12 months)
- exclusion record
- behaviour summary
- latest academic report
- SEN information
- option subjects and exam boards for KS4 pupils
- minutes from recent meetings (for example, LAC reviews, Team Around the Child meetings, school meetings)

The council primarily use your data for processing your request, this may be shared with Stockport Family and other government departments in accordance with our organisational privacy notice. We never sell your data to third parties or use it to market to you without your consent.
