You can use this form to report an alleged attack by a dog on another dog or animal.
Before you start
We’ll need you to provide some information to help us process the alleged dog attack you are reporting, such as:
- details of the incident
- photographs taken of the incident or animals involved
- the name and address of the owner of the attacking dog (if known)
- what happened after the attack
- your name
- your contact details
The dog warden service cannot action a report without all the relevant details, including those of the alleged attacking dog’s owner's identity and address.
How we use your data
The personal data that you provide is collected for the purpose of allowing us to undertake necessary actions to investigate reports relating to dog behaviour and attacks.
We have a statutory duty to investigate under the Dangerous Dogs Act 1991. Your personal information may be shared with the police or other agencies if the dog warden believes an offence has taken place, provided that it is lawful to do so.
We will retain your personal data for a minimum of seven years from the date the record was created.
