Make a referral to the School Nursing service

You can use this form to make a referral to the School Nursing service.

Before you start

You’ll need:

  • consent from the legal guardian for primary age children
  • consent from the child for secondary age children
  • details of the child
  • your details
  • details of who the child lives with (this is usually the child’s parent)
  • reason for the referral

The child you're referring must:

  • be of school age (from reception to year 11)
  • be attending a Stockport school, or live in Stockport and be home educated

You must be working with the child in a professional capacity.

How we use your data

We share your data with Stockport School Nursing service, a service within Stockport NHS Foundation Trust. It will be used to assess how best to provide the child, young person or family with the information, help or support they may require.
