Make a referral to the MASSH: urgent support or response

You can use this form to tell us your concern about a child’s welfare.

Members of the public

This form is for urgent referrals. You should use this form if you think a child may be at risk of serious harm, or if you think their basic needs are not being met. Otherwise, you can make a non-urgent referral.

If a child is at immediate risk of harm, call the police on 999.

Professionals

To use this form, you must:

  • have the consent of the family, unless this will put the child at risk of harm
  • make sure you understand the levels of need; this form is for making referrals at the level of urgent support or response

Before you make a referral

Subject access requests

Be aware that any information you submit via this form may be disclosed in a subject access request. If you have any concerns, you should note this on the form to let us know.
