Members of the public
This form is for urgent referrals. You should use this form if you think a child may be at risk of serious harm, or if you think their basic needs are not being met. Otherwise, you can make a non-urgent referral.
If a child is at immediate risk of harm, call the police on 999.
Professionals
To use this form, you must:
- have the consent of the family, unless this will put the child at risk of harm
- make sure you understand the levels of need; this form is for making referrals at the level of urgent support or response

Before you make a referral
Subject access requests
Be aware that any information you submit via this form may be disclosed in a subject access request. If you have any concerns, you should note this on the form to let us know.

We primarily use your data to assess and investigate reports regarding concerns raised about a child or young person. All information is processed in accordance with our organisational privacy notice. We never sell your data to third parties or use it to market to you without your consent.
