- make a referral to the emotional wellbeing hub or SEND hub
- tell Stockport Children’s Social Care about a child or young person being privately fostered
Members of the public
This form is for non-urgent referrals. You can use this form if you, a child or family needs extra support to meet their needs.
If you're referring someone else, you must have the consent of the family, unless this will put the child at risk of harm.
If you think a child may be at risk of serious harm, you should make an urgent referral.
Professionals
To use this form, you must:
- have the consent of the family, unless this will put the child at risk of harm
- make sure you understand the levels of need; this form is for making referrals at the level of getting more help

Subject access requests
Be aware that any information you submit via this form may be disclosed in a subject access request. If you have any concerns, you should note this on the form to let us know.
