This form is for non-urgent referrals. You can use this form if you're a parent or carer seeking support for a child.
If you think a child may be at risk of serious harm, you should make an urgent referral.
Professionals
To use this form, you must:
- have the consent of the family, unless this will put the child at risk of harm
- make sure you understand the levels of need; this form is for making referrals at the level of getting help

Before you make a referral
Subject access requests
Be aware that any information you submit via this form may be disclosed in a subject access request. If you have any concerns, you should note this on the form to let us know.
