CAMHS (Child and Adolescent Mental Health Service) is a Pennine Care Foundation Trust service that offers specialist support to children and young people who are experiencing moderate to severe mental health and emotional wellbeing difficulties.
CAMHS also provides support to families.
CAMHS currently accept referrals for children and young people who are 5 to 18 years old.
Who can use this form
You can use this form if you:
- are aged between 16 and 18 years old and wish to self-refer
- are referring a young person aged 5 to 18 years old, and:
- have consent from the legal guardian, if the child is under 16 years old
- have consent from the young person, if they're 16 to 18 years old

Subject access requests
