You can use this online form to make a personal injury or damage claim.
Before you start
To support your claim, you'll need to provide:
- information about the incident
- contact information
- up to 5, wide angled, colour photos that show:
- the damage
- the cause of the incident
- the location of the incident with some background for context. For example, a house, landmark or street light. If there's a street light nearby, you should make a note of the number on the column.
Photographs should be marked to show the:
- exact point where the incident happened
- direction that you were travelling
Personal injury claims
You can make a personal injury claim for yourself or on behalf of someone else if they lack legal capacity. This means, if they:
- are under the age of 18
or - have a mental impairment that affects their ability to make legal decisions
To make a personal injury claim on behalf of someone who lacks legal capacity, you must meet the requirements to be a litigation friend.
If you're making a claim for yourself, you must provide:
- your employment details, if you were employed at the time of the incident
- your National Insurance number
- your date of birth
If you're making a claim for someone who lacks legal capacity, you must provide:
- their employment details, if they were employed at the time of the incident
- their National Insurance number, if they're over 18
- their date of birth
- details about your relationship to the injured person
Personal possession or property damage claims
You must provide a copy of your invoice or 2 estimates for repairs.
Vehicle damage claims
You must provide:
- a V5 document to show proof of vehicle ownership
- your current MOT certificate, if your vehicle is 3 or more years old
- your current insurance certificate
- a copy of your invoice or 2 estimates for repairs
We cannot investigate your claim until we've received all the required documents.
