Who we are
We're made up of 5 social work teams:
- the AMHP team who complete Mental Health Act assessments
- the Working Age Mental Health community team who work with people over the age of 18
- the all age Mental Health Hospital Discharge Team who provide discharge planning to people leaving mental health wards
- the Older Adult community mental health team that works with people over the age of 65
- the Young Onset and Rarer Dementia team
What we do
- complete Care Act Assessments
- implement care where needed
- work collaboratively with our Pennine Care, NHS colleagues
- support the people of Stockport who live with complex or serious mental health problems
- provide support workers to people over the age of 18, who have complex needs
- provide tenancy support
How to get help and support from us
- call our contact centre on 0161 217 6029. Our team will triage your call and make sure you're put through to the right person to speak to
- you could also speak to your mental health practitioner
- if you're known to our specialist mental health service you can ask for a care act assessment
Care act assessment
We'll complete a care act assessment to find out what care and support you may need. Once we've agreed you have care needs you may receive support from us, or we may signpost you to other organisations. We'll also liaise with your mental health practitioner or any other relevant support networks you have.

- SPARC: a local voluntary organisation providing support to adults in the Stockport area who experience mental health problems
- Stockport and District Mind: an independent mental health charity that offers friendly, accessible support and information to promote wellbeing in the community
- Stockport Homes: as well as providing homes for people, we also provide advice and support for people in other areas of their life
- The Prevention Alliance: provide proactive and early support for anyone who is vulnerable due to their health, wellbeing or situation
- Welfare Rights Stockport: advice and support for benefits or debts
