What we do
Safeguarding
We put safeguarding children at the forefront of our practice.
We offer health assessments for all vulnerable children, including those subject to a child protection plan and those with a Team Around the Child (TAC) support plan where there may be health concerns.
The National Child Measurement Programme
All children in reception and year 6 are offered a height and weight measurement. Children in reception are also offered a hearing test.
This is to help us identify health needs early so we can support with early interventions if needed.
Visit the Transformation Stockport YouTube channel for a video about the National Child Measurement Programme.
School Nurse Health Clinics
This is a confidential service offering support and advice to young people around lifestyle issues such as:
- emotional health
- self-harm
- depression
- anxiety
- relationships
- weight
- puberty
- sexual health
Children and young people can self-refer. Referrals can also come from parents, carers or professionals (if they have consent from the child/young person and parents/carers where appropriate).
Continence
We offer assessments and support around daytime and night time wetting and any toileting issues.
Children in our care
We complete annual health assessments on all children in our care (meaning children looked after by the local authority). This may be in school or in the family home.
The parent or carer may not always be present, but we’ll always get permission from them before completing the assessment.
Support for children with complex medical needs
We co-ordinate and review individual health care plans for children with complex medical needs such as:
- diabetes
- epilepsy
- anaphylaxis or severe allergies
We offer medical needs awareness teaching sessions for staff in schools, in line with the Managing Medical Conditions in School Policy (2019).
Support for home educated children
We offer support around health needs for children who are electively home educated.
