Within the Equipment, Adaptation and Sensory team we use the Person-Environment-Occupation-Performance model (PEOP model) and it aligns with our strengths and asset-based approach. This means we focus on what matters to people, what personalised outcomes they want to achieve and we enable them to find the best solutions for themselves. It’s about self-determination and enabling people to live the lives they want to lead.

We’re building an Adult Social Care system in Stockport that’s based on outcomes, relationships and what matters most to the residents we support. We know that to do this we have to work closely with our residents, building a strong workforce who are able to work in a strengths-based way, to make sure the quality of the work we do with people means they are able to thrive.

In the Equipment, Adaptations and Sensory Service, we're putting this vision into practice as part of the Prevention, Wellbeing and Independence Business Group. Occupational Therapy is at the forefront this service delivery with a focus on prevention and early intervention through the provision of equipment and adaptations and strong partnership working. We achieve this by working with colleagues and partners to identify a person’s strengths and resources and support people and their carers to identify interventions that best meets their needs, goals and outcomes.

We work closely with partner agencies and providers, primarily under the framework of the Care Act 2014, DFG Legislation, Manual Handling Operations Regulations 1992, as amended by the Health and Safety (Miscellaneous Amendments) Regulations 2022 and any other relevant legislation. The principle of promoting and supporting wellbeing is at the heart of our practice.
