Stockport Council Adult Social Care team shortlisted for Smarter Working Live award
Stockport Council is celebrating after its adult social care team was announced as shortlisted in the Smarter Working Live awards.
National awards which celebrate the commitment of organisations and individuals to drive positive change, the Smarter Working Live award winners will be announced at a ceremony at Emirates Old Trafford on 23rd March.
The adult social care team is shortlisted in the ‘Outstanding Smarter Working Leadership’ category, for their work on adult mental health discharge processes.
The adult mental health discharge team is a team that has been developed to ensure that people with a severe and enduring mental health diagnosis are looked after from admission through to discharge. Previously, discharges were complicated and fragmented, and this was leading to long stays in hospital and frequent readmissions.
The team took the approach that physical and mental health are intertwined, and adopted a whole person approach to care.
Since the inception of the team, residents affected and their families have reported a real difference in the support available, as well as the quality of leadership and collaboration among social care colleagues and health professionals.
Councillor Keith Holloway, Cabinet Member for Adult Social Care and Health, commented: “It’s a real privilege to have been recognised for our work by being shortlisted for a Smarter Working Live award. Often our teams who do business as usual work are the unsung heroes of our community, but they make a real and tangible difference to people’s lives. Since setting up the adult mental health discharge team we have allowed vulnerable residents to return to their home and the community with the right support to reduce readmissions to hospital. Good luck to all the team attending the awards ceremony, let’s hope Stockport scoops the top prize.”
