Making a referral
Referrals can be made by a young person, their family or any other professional working with them. A referral can be made for a young person from the age of 16 but we can only provide support from the young person’s 18th birthday. If the young person needs support before then, you’ll need to make a referral to children’s social care.
The young person must give informed consent for a referral to be made.
Mental capacity
The Mental Capacity Act 2005 is an important piece of legislation that's relevant to anyone aged 16 and over. If the young person’s capacity to give consent is questioned then a mental capacity assessment will be needed.
Capacity is decision specific and a young person may have the capacity to make some decisions but not others. The underlying principle of the Act is to make sure that young people and adults are supported to make as many decisions for themselves as possible. If they're assessed as lacking capacity, any decision made or action taken on their behalf should be done in their best interests.
The decision maker could change for each decision and the family’s views are taken into consideration.

How to make a referral

The referral will be sent to the Transitions team and the duty social worker will call you. After we’ve spoken to you, we’ll send you a letter to confirm that we’ve received your referral and the young person will be placed on the Transitions waiting list.
If we cannot get in touch with you by phone, we’ll send a letter asking you to contact us. If we’re still not able to contact you, we’ll contact the referrer to let them know and the referral will be closed.
