Overview
Making Every Adult Matter (MEAM) is an approach that works across multiple agencies and organisations.
Who it's for
MEAM aims to help people with multiple disadvantage. This means people who are facing a combination of problems.
You can make a referral for someone if:
- you're working with them in a professional capacity
- they're an adult aged 18 years or above
- they live in Stockport, or are without a fixed address but still in the Stockport area
- they meet the following criteria:
Experience of multiple disadvantages
They're facing a combination of interconnected issues, including but not limited to:
- homelessness, or risk of homelessness
- substance misuse (drug and/or alcohol dependency)
- complex mental health challenges
- contact with the criminal justice system
- domestic abuse
Complex needs
They have complex support requirements that are not adequately addressed by existing services.
Service engagement challenges
They have historically faced barriers in accessing or engaging with traditional support services.
Consent and willingness
They consent to participate and are willing to engage with the support offered by the service. Public duties to support safety and wellbeing may at time override this consent. Safeguarding considerations will always be given to interventions.
How this works in Stockport
In Stockport, the MEAM Hub is a weekly multi-disciplinary team meeting. It includes:
- Adult Social Care services
- British Red Cross
- Pure Innovations
- Stockport Homes
- Stockport Support Hub
- The Wellspring
They also work in partnership with:
- the NHS
- Greater Manchester Police
- mental health services
- probation services
- the third sector (charities, social enterprises and community groups)
These organisations work together as part of the MEAM Hub to directly improve the lives of people facing multiple disadvantage.
Additional considerations
Inclusivity:
- the service must be accessible to all eligible individuals, regardless of gender, race, disability, or other intersecting identities
Flexibility:
- recognise that individuals may not fit neatly into predefined categories; apply professional judgement to assess eligibility on a case-by-case basis
