Social care team
If you have a learning disability and live in Stockport you can self-refer to the learning disability team to request a Care Act assessment. You, a family member or friend can do this by calling Adult Social Care on 0161 217 6029.
When you contact us
We'll ask you some questions to decide if the learning disability team is right for you. You may speak to a duty worker who'll take some information about you and your care needs.
If you have a learning disability, a social worker from the learning disability team will do a Care Act assessment with you. They may visit you to do this or could arrange for you to meet them at a council building. Visit our Getting Care section for more information on what happens during the assessment.
