The One Health and Care Plan

Background information

Stockport, like other local areas across the country, is facing a number of issues with health and social care. This means that we need to change how these services are delivered.

This includes:

• with an ageing population facing more complex care needs and heightened risks of loneliness and isolation, it's crucial to strengthen preventative approaches, build inclusive services and communities that promote connection and wellbeing.

• rising birth rates in deprived communities shows the increasing need to address inequalities affecting children and young people.

• understanding how the increased cost of living and wider economic challenges affect our most vulnerable residents, and working to reduce those impacts

• changes in the most common preventable health issues experienced by the population that are linked to health behaviours.

• a need to bring services closer together to make them easier to access and support individuals’ needs in a joined up way.

• too many people are admitted to hospital and we need to provide more care closer to home.

• with increasing demand on services and forecasted deficits, it is essential that Stockport builds a more resilient and sustainable system

The One Health and Care Plan 2024 to 2029

The One Health and Care Plan (OHACP) is a single plan for health and care from 2024-2029, developed through extensive engagement with our local people. It builds on existing organisational strategies with the aim of improving support to local people through increased collaboration.

As one plan for health and care, this document has replaced the CCG Strategic Plan, Stockport’s Health and Wellbeing Strategy and our Locality Plan under the GM ICS Strategy, GM Joint Forward Plan and the One Stockport Borough Plan.

The plan sits within the context of major national change in the organisation of the health service. The Health and Care Act 2022 established Integrated Care Systems (ICS) to deliver joined-up place-based working across health and care providers. It is focused on how we continue our transformation of the local health and care system as part of the Greater Manchester ICS, and crucially how we translate that in Stockport, with our Locality Board, and in our neighbourhoods.

The One Health and Care Plan is intended to be a high-level strategy outlining our ambitions for how we deliver that transformation. Each priority will be underpinned by detailed annual delivery programmes that will follow agreement of the strategy. Each of these delivery plans will be accountable to the Locality Board through agreed governance.
