About the app
The Greater Manchester Early Years App is a digital platform that allows your Health Visitor to share development assessment forms, known as ages and stages questionnaires (ASQ forms), with you electronically, instead of using paper forms. This:
- reduces delays with postage
- reduces waste
- gives you instant access to all of your child's development assessments in one place at any time
The app is linked to your NHS account so you can be assured that it is a confidential and secure platform.
How to access the app
The app is currently being used for children under the age of 2 and a half years in the following areas:
- Bramhall
- Cheadle Hulme
- Gatley
- Heald Green
The app will go live in the following areas in June 2025:
- Adswood
- Edgeley
- Great Moor
- Offerton
- Hazel Grove
Parents and carers of children aged under 2 years in these areas will need to confirm their email address and phone number with their Health Visitor. They'll then receive an invitation to register for the early years app.
We can only link one parent or carer to the app so you should consider who will be responsible for completing the questionnaires. You'll need an NHS login to access the app.
For more information about the Early Years App and how to complete your child’s digital ASQ forms, you can:
There are plans to extend the use of the app to the whole of Stockport.
