3. Your care and support plan

If your assessment shows you're eligible for care and support, you’ll receive as much, or as little help as you need to arrange the support you want to meet your agreed outcomes.

We'll also provide information about the different services and support that are available. This will include free and low cost services from local independent and community organisations.

Your care and support plan will include:

• details of the help you'll get and who will provide it
• how often you'll get help
• contact details

Financial assessment

We can carry out a financial assessment to work out how much you may have to contribute towards the cost of your care.

We'll also make sure you're receiving all the benefits you're entitled to and provide advice on where you can get independent financial advice.
