We are a community rehabilitation service comprising of physiotherapists, occupational therapists and assistant practitioners that works closely with the Steady in Stockport falls service.
Our therapy staff work together with the patient, the local neighbourhood teams, other professionals, family and carers to provide person-centred, goal-orientated care. We help adults with a physical disability to achieve their optimum level of functional independence in all aspects of daily living, in their place of residence.
Our work includes:
- Advice on management of long-term conditions for patients and their carers.
- Assessment and intervention to improve mobility and reduce the risk of falls.
- Provision of standard indoor walking aids.
- Walking aid clinic based at Kennedy Way in Stockport which takes place on Monday afternoons by appointment only. Referrals can be made via a GP or other healthcare professional. Self-referrals are accepted for walking aids if the patient has been seen by ACTT within the last six months for the same problem.
- Assessment and intervention to optimise independence in daily living activities e.g. kitchen and self-care tasks.
- Provision of simple and complex activities of daily living equipment as part of a rehabilitation programme from the Millbrook and local equipment stock.
- Advice and demonstration of techniques for self-management.
Eligibility criteria You are eligible if you:
- Are an adult aged 18 years and above.
- Have a physical disability registered with a Stockport GP. Written referrals are accepted from any health or social care professional. It is expected that the referrer has had face-to-face contact with the patient, the only exception being in the case of a patient’s request for a walking aid. The patient must consent to the referral and be aware that it has been made.
