- enabling people to live independently: Supporting people to live independently is vital for autonomy and wellbeing, requiring a holistic approach across health, safety, social support, and accessibility
- commissioning for early help and prevention: Early help and prevention are central to commissioning strategies, with a focus on maximising independence
A preventative, independence-focused approach should underpin all interactions in adult social care, from engagement with universal services and community groups to requests for information, assessment, planning, review, and strategic development. The wellbeing principle and strengths-based practice is central to practice, commissioning and good conversations with people.
To achieve our vision, we must work in partnership to deliver sustainable, measurable benefits for the residents of Stockport. Underpinning our work is the Stockport Live Well programme.
