Raise a quality issue or concern about a care provider

If you have a quality issue or concern about a care provider we always recommend that you speak to the care provider first. In most cases they'll be able to resolve the issue or concern directly with you.
If you're still not happy, get in touch with us by emailing email@example.com.
You'll need to tell us:
- the name of the service user and their contact details
- your name and contact details
- the name and address of the care provider
- details of the concerns and dates, if known
We'll respond promptly and advise you on what happens next.
